To be eligible for support coordination services funded by the National Disability Insurance Scheme (NDIS) in Australia, individuals must meet specific criteria. Here is a detailed checklist:
The applicant must be under 65 years old at the time of application.
The applicant must be an Australian citizen, a permanent resident, or hold a Protected Special Category Visa.
The applicant must have a permanent and significant disability that affects the ability to perform daily activities and participate in society. The disability can be intellectual, cognitive, neurological, sensory, physical, or psychosocial.
The disability must substantially reduce the individual’s functional capacity to manage daily activities, navigate support systems, and engage in social, economic, or community life without assistance.
There must be a demonstrated need for support in navigating, coordinating, and managing NDIS and other services. This need is often identified during the NDIS planning process. Support coordination should be outlined in the individual’s NDIS plan, specifying the types of coordination required and the expected outcomes.
Documentation from healthcare professionals confirming the nature and impact of the disability.
An NDIS plan or another formal support plan detailing the need for support coordination services.
Service Navigation: Assistance with understanding and navigating the NDIS and other support services.
Plan Implementation: Support in implementing the NDIS plan, ensuring the individual accesses the services and supports included in their plan.
Capacity Building: Helping individuals build their capacity to manage their own supports and achieve greater independence.
Crisis Management: Support in managing and resolving crises or complex situations that may arise.

Provide necessary documentation and evidence as part of the NDIS application.
Collaborate with an NDIS planner to include support coordination services in the individual’s NDIS plan.
