5. Need for Support Coordination:

There must be a demonstrated need for support in navigating, coordinating, and managing NDIS and other services. This need is often identified during the NDIS planning process. Support coordination should be outlined in the individual’s NDIS plan, specifying the types of coordination required and the expected outcomes.

Medical Evidence:

Documentation from healthcare professionals confirming the nature and impact of the disability.

Support Plan:

An NDIS plan or another formal support plan detailing the need for support coordination services.
